Who Can Apply
- Age: 18–100 years
- Residence: Uttar Pradesh residents only
- Deceased person must have been an elected representative in the Panchayat system
- Death must have occurred during the term of office
- Applicant must be a legal heir or dependent of the deceased representative
- Applicable only in Uttar Pradesh
